Ministry of Sound provide the PrePay Maestro® Card is available to UK residents over the age of 18 - although it should be available to over 13's in the near future.  Choose your card design, order online or via SMS.  You will need ID and a credit or debit card to sign up but there will be no credit or employment checks.  This PrePay Card is not a debit or credit card - just top up with a value, then spend until it runs out.  It costs more to apply via SMS - activation costs £5.49 in total so it might be better to apply via the Ministry of Sound website.  Charges associated with the card include £1.50 for ATM cash withdrawal and 3% on foreign transactions. 
 
UK transactions incur a 20p fee per transaction.  With the Ministry of Sound PrePay Card you can get yourself and 3 friends on to the Ministry of Sound guest list once a year (applies to over 18's only).  You can also earn free downloads with a certain amount of top ups.
